Difference between "fakeer" and "miskeen"

What is the difference between "fakeer" (فقير) and "miskeen" (مسكين)? Are they different levels of poor, or do they refer to the same thing?

Also what conditions must apply to be considered for each of the two with respect to sadaqah/zakat?

إِنَّمَا الصَّدَقَاتُ لِلْفُقَرَاءِ وَالْمَسَاكِينِ وَالْعَامِلِينَ عَلَيْهَا وَالْمُؤَلَّفَةِ قُلُوبُهُمْ وَفِي الرِّقَابِ وَالْغَارِمِينَ وَفِي سَبِيلِ اللَّهِ وَابْنِ السَّبِيلِ ۖ فَرِيضَةً مِّنَ اللَّهِ ۗ وَاللَّهُ عَلِيمٌ حَكِيمٌ
